Please take a few minutes to not only complete the survey, but to pass this link on to others in your contact list that live in the Ottawa County area. Our community partner, Integris Health, has been charged with completing community needs assessments in each of the areas they have a presence. Because we are so fortunate to have Integris Baptist Regional here in Miami, our Community Wellness Committee was asked to assist Integris in the distribution of the surveys (to adults 18-over) throughout Ottawa County. The data collected from the surveys will be compiled and analyzed to determine the 5 most significant issues impacting the health of our county. Integris will then work with the community coalitions to develop a community health improvement plan for Ottawa County that Integris Baptist Regional will implement to focus on and address those health concerns identified by the community.
